Do it yourself.

First, carefully cut the thread off the guide by locating the guide foot, then cutting SLOWLY along the foot up to the head of the guide.

Remove, pick off all thread, heat with a hair dryer, and work off the old epoxy. Your thumb nail works.

Then take the guide, go to Cabelas, Janns, Barlow and located one of similar size. They have ring size charts and colors.

Then take a thin strip of masking tape, tape the guide, and wrap it. Use a cardboard box w/ two triangular cuts on opposite sides to hold the rod, and a book to hold tension on the thread. Go to any rodbuilding website for tips.

Cabelas sells 2-part epoxy for rods in individual packs. You'll need something like flex coat that will not yellow in weather and is flexible.

If you're close to No VA, I'll do it for a beer a guide plus parts.
